eBay. Buy now for 120€.
You can download the original embedded system from Intel, at most you need a DOM and an USB stick to make it work. OR Windows Home Server.

I took a quick look and even found a 5750 passive for under 100€. It has two DVI ports which could be a bonus.
The 440GT is the fastest nVidia model I could find.
I don't know which card will accelerate your 3D modeling program.

Do you want power efficient or cost efficient? I think most of the suggested systems are overkill. If you don't want to use the system for playback or transcoding I would go with a ready made system. Whether or not you can under volt the CPU depends on the mainboard's bios (or UEFI) options. You have to do a bit of research yourself, there are too many different boards to give a general answer. Xbitlabs is a good source for reviews. I would advise against a G620T, since it lacks certain features...

The case is pretty open if you use a Pico PSU. The best cooler for fanless operation is the Thermalright HR-02. It should cool a dual core Sandy Bridge completely fanless, but it's expensive. Check out the Thermalright Silver Arrow. It comes with two great fans and is usually cheaper then the Noctua.
Also, you should check out the MSI R6850 Cyclone. It's way quieter under load than the standard model and you can flip a switch on the card to set the fan to whisper quiet.

If you must have a new cooler for two fans the Noctua NH-D14 and the Thermalright Silver Arrow are your best options. I recommend Thermalright (I always do and use them exclusively) as the fans that come with the Silver Arrow are better than those of the Noctua NH-14D. You shouldn't expect too much ...

How about option 3: replacing just the fan?
The Zalman cnps 10 is a good heatsink, the only problem is the loud fan. Get a Scythe PWM fan and some zip ties.
