Someone was helping with a screw that wasn't budging and ended up scratching the lower part of the heatsink. This is to the side, though, but a pretty big scratch. If it's not around the area where the die would be (old school amds so no cover), will it matter? Can it affect temps?

You should be fine, if the scratch doesn't interfere with the CPU-->HS contact area.
