I want to add an 80mm fan to a headphone amplifier that I'm building, mounted maybe on the top. The amplifier (β22) is very sensitive to noise (the electrical kind) and interference, and I was wondering if anyone has any knowledge as to whether a fan would introduce any ill effects to the circuitry. Maybe some bearings are better than others?

The case that I will be cooling will be an 8.5" x 6" x 3". I really won't need much cooling power, so 5V may be enough.

Don't know about the fan. My guess would be yes it will introduce some noise, especially if powered from the same source as your amp, but the only way to find out for sure is to experiment.

How about a metal case/offboard heatsink?

I'm not familiar with the β22. Will it really produce that much heat? Or is your ambient sky high?
