That shouldn't be that difficult these days. I did it 5 years ago with a PicoPSU, an SSD, Speedfan to control the fans and a AMD 89W TDP CPU. And today Intel usually do not get to the rated TDP, and with speedstep and some undervolting you should not have any issues. Make sure to find a semi-fanless...

For a CPU to work passive you need a big heatsink and not more than 20W at load. I don't agree.
