Have you thought about trying to find a micro-atx case that can be modified to fit full atx? I'm thinking that may not be the worst idea yet. Perhaps a desktop so the optical drive can sit over the motherboard?

Slightly against cases with fans of sizes over 120mm, there's no replacing them. You may find some 140mm fans, but 200mm, I strongly doubt it.
