How about this one?

The only thing is that, while AC bill it as a 120x38mm fan, it has the same airflow and rotational speed as their common 120x25mm fan. The major difference being that the 38mm version is in fact mounted on a special frame on rubber mounts. Interesting design, though. I use the louder, 1500rpm version, and at 5v it's pretty silent, though hardly dead quiet.

I believe Felger Carbon likes his 120x38mm SilenX fan, but I have looked into this a little bit and I believe he may be the only (admitted ) user of that particular fan currently.
If nothing else they at least run at slower stock speeds than the Panaflo, so that would give you a better place to start....
Try a search with his user name and see what turns up.

BrianE, I am a new member here (but have been reading this site for over a year) so you guys don't know me yet, but in my case I have 3 120mm Silenx Ixtrema 14dBA fans, and I think I can tell you why not many people have (or admit to having) these fans, each one of these 3 fans cost me $30! That is a TON more than any other fan and to be honest I was stupid to pay this much for them, but barring the price they are very nice fans and they came with both screws and silent rubber mounts as well as 3 pin and 4 pin Molex connections which are a nice touch, though they are definitely not 14dBA at 12v, more like 30-35IMO (the fans SilenX now sells are the Ixtrema Pro series, I have the older Ixtremas which you seem to be talking about [there is no 38mm thick version of my fan, for instance] ).
The sticky thread Tibors linked also talks about the AC fans, Elixer.
There's a 38mm version of the YateLoon, but I don't know of any vendors. Perhaps silencio could fix you up with one. They don't make a slow version though, medium being the slowest, and based on a quick comparison of the manf. specs it doesn't sound too quiet either.
Manf. Specs Comparison:
D12SL-12: 120x120x25, 12v, 1350rpm, 47.0cfm, 28dB
D12SM-12: 120x120x25, 12v, 1650rpm, 70.5cfm, 33dB
D12SM-12D: 120x120x38, 12v, 2300rpm, 96.0cfm, 43dB

Maybe you could mod a D12SL-12 motor into a D12SM-12D.

Hmmm, anyone has one of this? May be interesting... Seems to actually go as low as 600rpm and you can boost it up to 2400rpm for extreme (overkill) overclocking if you ever need it.

Oooh, I like that blade count on that Silverstone. Extra blades + extra thick could = extra good in restrictive environments -- if it turns out to be a solidly quiet fan.
Can't wait too see the testing results of it here, but based on the manf. specs it is louder that its not particularly good 25mm sibling.

what about this guy?
Panaflo 120x120x38mm, 68.9 CFM @ 1700 RPM, 30 dBA
They have 4 of them, but the quiet one is the one above. Also don't enlarge the pic. and expect the photo to be accurate, they mix up the photos

I don't understand when you said identical spec.. I thought all 4 fans has different spec. From the far left to the far right, they all different:
120x120x38mm, 68.9 CFM @ 1700 RPM, 30 dBA, 2.16w, 180 mA, 12v DC, 3 pin Panaflo tail to standard 3-pin connector included. Our current stock of this model does not support TACH output.
120x120x38mm, 86.5 CFM @ 2100 RPM, 35.5 dBA, 3.36w, 340 mA, 12v DC, 3 pin Molex with tachometer support.
120x120x38mm, 103.8 CFM @ 2500 RPM, 41.5 dBA, 5.52w, 460 mA, 12v DC, 3 pin Panaflo tail to standard 3-pin connector included. This model is BX and does support TACH output.
120x120x38mm, 114.7 CFM @ 2750 RPM, 45.5 dBA, 6.12w, 510 mA, 12v DC, 3 pin Panaflo tail to standard 3-pin connector included.
